The Department of Graduate Medical Education requires that each student complete an on-line application for each rotation. The application must be submitted no later then 45 days before the start date of the rotation.

 

 

Each application will be reviewed, however requests are not guaranteed. You will be notified by email of the approval or denial of the rotation. If you are approved, we will need from your school as confirmation:

1. Letter of good standing
2. Proof of malpractice
3. Immunization records
4. Photograph

 

 

Upon we receipt of this documentation; you will receive confirmation by email. At that time, we will provide you the information you will need to begin your rotation at South Pointe. If you have any questions, please contact us at: gme@cchseast.org.

 

 

If you need housing, please mark the application appropriately. Our facility has dormitory style housing on the campus of the hospital. The beds are made available as a first come, first served basis. You must provide your own linens. Meals are provided in the cafeteria as follows:

• Breakfast $3.00 limit
• Lunch $7.00 limit
• Dinner $7.00 limit
